Custom forms
Build company forms in the Library, decide who fills them, and review answers under Submissions.

The Forms module (sidebar → Operations → Forms) lets you build your own forms — incident reports, member assessments, shift checklists — and collect answers from staff on the web or drivers in the app.
The Library
Forms → Library lists every form with its Form type, Category, Fill roles (who can fill it), Status, and when it was last updated. Filter by search, form type, or category.
Creating a form
- Click Create form.
- Give it a Form name (for example "Vehicle incident report"), an optional description, and an Access level — who this form is for. You can refine where it appears later in the builder.
- Build the form's fields in the builder and save.
Managing forms
Each row offers:
- Edit — open the builder
- Fill out — complete the form right in the portal (driver-app-only forms are filled from the app instead)
- Archive / Restore — retire a form without losing its submission history
Forms drivers see
Forms aimed at drivers surface in the driver app. Forms categorized as pre-shift or post-shift open automatically around clock-in and clock-out — drivers see them under Shift Forms with a completed count (see Driver timekeeping).
Note: Vehicle inspections are their own dedicated system with per-driver targeting and PDF logs — use Inspections, not a custom form, for pre/post-trip vehicle checks. See Shift inspections.
Submissions
Forms → Submissions collects every submitted form across the company in one place. Open any submission to see the full response, who submitted it, and when.
Good patterns
- Incident reports — a driver-fillable form means the details are captured at the scene, time-stamped, before memories fade.
- Member assessments — standardize intake evaluations so every office staffer collects the same information.
- Post-shift condition reports — a lightweight end-of-day form (fuel level, cleanliness) that complements the formal inspection checklist.
Keep the required fields short — forms that take under a minute get filled honestly; ten-minute forms get filled at the end of the week from memory.
